New life insurance premium not present in Form-16 : Is there any way to rectify the Form-16?

I forgot to update about the new life insurance premium in investment declaration to my company’s finance department and now the Form-16 does not show that premium amount. Is there any way to rectify the Form-16?
Should I go ahead with filing Income Tax Return?
Do I need to submit any additional document with Income Tax Return?

While filing the Income Tax Return you can claim for life insurance premium rebate and the I.T. officer will access and refund you the eligible amount later on. But you are asking it too late the I.T. Return date for salaried persons is already gone (31st July).
