Age for medical tests would vary from company to company.
Normally three tests are taken by the insurance company e.g. E.C.G., Urine and Blood Sugar test but the required tests may be change from company to company depending upon the type of case.

I have taken insurance from National Insurance Company and they have asked for three tests -
1 - E.C.G.,
2 - Urine and
3 - Blood Sugar test

Although there are no specified list of Doctors/Clinics/Hospitals/Labs to get the medical exam but some insurance companies like "New India Assurance Co. Ltd." has its own list of hospitals from where medical exam has to be done and these tests are to be verified from the Doctor M.D. only. Please check with the insurance company you are going to buy insurance.
