Forms for Switchover of the funds in ULIP Policy?

Is there any specific forms for ULIP Units Switchover between different funds ?

Write a request on simple paper and submit in LIC branch.

1- Write an application on Fund Switch
2- Mention Policy No. and Exact details of fund switching details e.g. the fund you wanted to exit and the fund you want to enter.
3- Sign it and submit to LIC Office
