Non receipt of Health Insurance ID card

I have not recieved health insurance ID card, which is required for cash less treatment. Could you please tell me whom should I write - TPA or Insurer.

Please advice.

Write a letter to TPA Head Office mentioning -

1. Your Mediclaim Policy No.,
2. Insured name,
3. Insurance company,
4. Reference No (If any)

Ask them to send the ID Card.

Now the e-card is available online on the TPA - websites. Just log on and take a print out. Even if you face any problem with wrong data on the e-card, just raise online request.
