The Service Tax or Stamp Duty paid on medical insurance premium is not allowed as a deduction under section 80D. Deduction under section 80D can be only claimed on the amount paid as Medical Insurance Premium and not on the service tax levied or Stamp Duty thereon. So option 3 in your query is correct.
