Question Credit Card - PA - Insurance Claim procedure?

Hi All, With each credit card, we usually get some free P.A. insurance cover. Do anybody has any idea - How to claim that if some thing happens to us? Please tell me in layman language?

If you know the insurance company from where the credit card company had taken the policy, than you have to submit below documents to them. If you don't know. Please call the customer care of the credit card company and take this info from them. Please submit credit card info like - past statements and photocopy of credit card (both sides) and following documents to the insurance company -
    A. A notice of death should be provided within 30 days of the credit card-holder's death. This notice should include a brief description of the accident.
    B. The claim must be filed within two months of the loss of life due to an accident and the following notarised documents should be submitted along with the claim form -
    1. Death certificate
    2. Police report/ panchnama
    3. FIR
    4. Post-mortem report
    5. Examining medical officer's report
