Pre-existing conditions, how to cover?

I have some Pre-existing conditions, which I wanted to cover. Please suggest - how to cover the same?

You have to declare all the pre-existing conditions on the proposal form and after 4 years waiting period, it will be covered by most of the health insurance policies.

Is there any policy in which I can get it covered from day-1, after paying extra premium?

There is no policy other than group health policies. Some policies cover pre-existing conditions from 2nd year onwards.
