How to reinstate PPF A/C?

Your account can be revived on payment of a fee of Rs 50 for each year of default (50*3 = Rs. 150), along with the arrears of subscription of Rs 500 (500*3 = Rs. 1500), for each such year.

Is there any reinstatement form for the same? Or we just need to pay the above penalty and give an application with it.
