PAN card application - what address proof will be accepted?

2. Bank account statement/ bank pass book
3. Ration card
4. Passport

Proof of Address

1. Bank account statement/bank pass book
2. Passport
3. Ration card

Do we need to apply with two copies of same documents like Ration card (one for proof of identity and one for address proof)?

One copy will work, But I would go for two different set of proofs.
