To obtain duplicate policy documents, you need to fill out a 'Duplicate Policy Request' or Application form and send to your insurance company.
You will have to pay the charges for the issuance of the duplicate policy ex - charges for stamp fee. Some insurance companies need additional requirement like FIR copy/advertisement in the newspaper. So please check with your insurance company.
