Please read the terms and conditions of the proposal form or other documents. In normal condition if you consume Tobacco/Smoking in any of the following then you are Tobacco User -
    (i) Smoke / Consume Or Have You Ever Smoked/Consumed Cigar, Cigarettes, Beedies Or Any Other Tobacco Products (Pan Masala Etc.)?
(ii) Have You Ever Sought Or Been Given Medical Advice To Reduce/Abstain From Smoking/Tobacco Consumption?

Usually, if a person has quit smoking for 3 years or more, he's considered a non smoker. However it is best to inform insurer about the same to avoid any misunderstandings.
Everyone has a scheme of getting rich.. Which never works.

Depends on the policy, but just for example, the Kotak Life Insurance proposal form mentions that the client has to declare whether he was a smoker or drinker earlier also even if he has left that habit long ago. Also, it depends on the company whom they consider as a non-smoker at the time of issuing a policy. For example: Max New York Life Insurance, for its Platinum Protect (term insurance), considers people, who have left smoking more than three years ago, as non-smokers. Please check the company’s rule.

It's very from company to company. As per MetLife's Met Protect Non-Tobacco/Tobacco User Means -
    # People who have not smoked or consumed tobacco or nicotine products in any form in the last 5 years (Tobacco product includes but not limited to Cigarettes, Bidis, Cigars, Chewable Tobacco like Ghutka, flavoured Pan Masala etc.) and who have tested negative for the Cotinine Test. The special premium rates will be offered subject to satisfactory results of "Cotinine Test" besides other underwriting requirements as may be required by the company from time to time.
