    Exclamation Lost my original policy document

    Hi All,

    I am have taken LIC - Jeevan Aastha policy. I have got transfered to Nagpur from Kolkata. During transfer I have lost some of my documents, with this I have also lost my original policy document. Could you please tell me -
    1 - How to get a duplicate policy document in case I lose the original?
    2 - Is there any charges for the same?

    To obtain duplicate policy documents, you need to fill out a 'Duplicate Policy Request' form and send to your insurance company.
    You will have to pay the charges for the issuance of the duplicate policy ex - charges for stamp fee. Some insurance companies need additional requirement like FIR copy/advertisement in the newspaper. So please check with your insurance company.

    Write an application to the branch mentioning your policy no and narrate the circumstances how it was lost. Better take FIR, so that nobody can misuse it.
