What is the procedure of PAN Correction?

I have just got my PAN card with misspelling of my name on it. Could you please tell me - What is the procedure of PAN Correction?

The applicant is required to fill all the columns of the form and tick the box(es) on the left margin of the appropriate item where change/correction is to be done.
